GROUP/COMPONENT NAME:

ÒBUILDING WELLNESSÓ

PURPOSE:

Our group purpose is to Òreach out and provide quality services (intervention strategy)Ó and our goal is:  to reach out to the community in need and assure access to effective, qualitative services as per the Informed Consent Form information sheet.

The Witness for Wellness is a research project developed by and for our community to develop approaches to mental wellness that can help overcome the emotional problem of Depression.  We in this group represent the community at large.  We must not be afraid to speak up, ask questions, to share with others, seek input from others (outside in the community) and bring the information back to the group to take in as information gathered.

ACTION PLAN:

Monica reinforced our purpose.  It was explained that we are to create an action plan with timelines; we will decide if we want to use the Action Plan presented to us.  There was extensive discussion on the Action Plan provided to us from the previous Orientation Meeting held on November 7, 2003.  The consensus of the group was for each member to select an Action Plan and give feedback to the group, at the next meeting.  There will be presentations made by each regarding the methodology and timeline for the plan chosen.  At that time we will decide where we want to focus our efforts.  Copies were provided to each member.

The plans were assigned numbers with a member or members responsible to bring information for the methodology and timelines back as follows ( see attached Action Plans):

1-    Monica and Rev. Johnson
